Auto-triggered post-visit. SMS within 5 minutes of note closure. Multilingual LLM Q&A (100+ languages design goal). Visit summary, appointment + refill requests, risk scoring (30-day hospitalization, fall, nutrition). Identifies high-risk patients for Care Management enrollment.

Referral Review
AI-assisted referral decision support. Two inputs, a guideline-concordant recommendation in under ten seconds. 400+ conditions, 39 hardcoded patient-safety escalations, immutable audit record on every recommendation. Validated production data shows 40%+ reduction in total referrals, appropriately.

TCM/CCM enrollment with cumulative-time billing capture. Work queue for high-risk patients identified across the platform — high-risk follow-ups from Patient Engagement, CM-qualifying referrals from Referral Review, and CM-eligible new patients from Enrollment. EHR-documented follow-through that One-Click is required to list at the next encounter.

Downstream reporting across six measurement categories: patient acquisition, financial performance, utilization impact, coding accuracy, risk adjustment, and network management. Planned white-label partnership with Ensure Data Solutions (EDS); optionality preserved.
